Responsibilities

• Delivering in-home care to patients with a variety of medical conditions.
• Developing and implementing individualized care plans in coordination with a multidisciplinary team.
• Monitoring patient progress and making necessary adjustments to care plans.
• Educating patients and their families on managing their health conditions and promoting overall wellness.
• Documenting patient care and progress accurately and timely.
• Collaborating with other healthcare professionals to ensure comprehensive patient care.
  
  

Requirements

• Current RN license in the state of Oregon.
• Previous experience in home health care or a related field is preferred.
• Strong assessment and clinical skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation.
