This is a “Field Based” position.

The Palliative Care Nurse will incorporate the understanding of palliative care philosophy and practice into the role of coordinating the care of palliative care patients. The Palliative Care Nurse will provide guidance and support to other palliative care team members in the overall delivery of services related to palliative care.

Duties:

  • Assess the treatment and management of pain and other symptoms related to palliative care patients.
  • Provides ongoing assessments of the impact of life - limiting chronic illness on the patient’s physical, functional, psychosocial and environmental needs.
  • Establish and implements an individualized plan of care for all palliative care patients based upon patient care standards, palliative care team input and patient/family preferences. Plan of care is reviewed on a regular basis.
  • Initiates appropriate preventive and rehabilitative nursing procedures when appropriate.
  • Ability to conduct an advanced care discussion with a patient and their family and properly document their wishes in the electronic medical record.
  • Determines the scope and frequency of services needed based on acuity and patient / family needs.
  • Uses the case management approach and refers to other services as needed.
  • Respect and assess for the spiritual, psycho-social, cultural and religious preferences of the patient and family.
  • Assess for caregiver burnout, and provide community resources when appropriate.
  • Serve as the coordinator for the palliative care team and assists in all facets of care coordination for palliative care referrals.
  • Consults and educates the patient / family and other caregivers regarding the disease process, pain and symptom management, end of life care and processes for dealing with issues of ethical concern.
  • Consult and collaborate with community physicians on palliative care services, promoting teamwork with physicians, staff and patients and promotes excellence in care through education, mentoring and collaboration.
  • Develops and maintains effective relationships with all key stakeholders necessary for the success of both the individual case and the program.
  • Maintain and review patient records, charts and documents to support accuracy, quality measures, and best practice in palliative care.
  • Document in a consistent, complete, thorough and legible way. Documentation includes the outcome of communication, assessment and plan of care for patient.

Requirements

  • Registered Nurse with a current license to practice in the state of Oregon.
  • Minimum five years of recent clinical experience as RN, 2 years of hospice, oncology and/or home health preferred.
  • Current Oregon driver’s license with access to reliable transportation with proof of current automobile insurance.
  • Proficient knowledge of palliative care and end - of - life symptom management, Certified Hospice and Palliative Nurse preferred.
  • Excellent verbal and written skills.
  • Excellent organizational and prioritization skills
  • Ability to interact productively with individuals and with multidisciplinary teams.
  • Proficient computer skills, including Microsoft Word, Excel, and Outlook.

Preferred Qualifications:

  • Knowledge of palliative and hospice medicine

Physical & Mental Requirements:

  • Ability to lift up to 100 pounds
  • Ability to sit for extended periods of time 
  • Ability to use fine motor skills to operate office equipment and/or machinery
  • Ability to receive and comprehend instructions verbally and/or in writing
  • Ability to use logical reasoning for simple and complex problem solving
